Our office now has a patient portal for you to access your health records. It includes most labs, encounter notes, requesting appointments and prescription refills. If you have any abnormal test results, you will need to contact our office to get that information from us. To access your health information give our office a call and we can provide you with your 4 digit to get registered today.
We ask that if you are needing a prescription refill then to please contact your pharmacy for them to send us a request, or using the patient portal to let us know what you are out of.
